Primary Swimming Carnival Nomination Form

Date: Tuesday 2nd December 2025

Venue: 2nd War Memorial Aquatic Centre, Rockhampton 

Time: 11:00am - 2:00pm

Please nominate your child in the following swimming events listed below by checking the box for each event.

You must complete a separate form for each student.

•  Age is what your child/ren have or will turn in 2025.

•  Students aged 9yrs to 12yrs can nominate to swim either 25m or 50m for each stroke – not both.

•  Under 8 students wishing to compete in the 25m need to be aware that it is an unaided race and a stroke of choice.

•  Under 8 students are to compete in the U/8 activities.
